What is HoLEP (Holmium Laser Prostate Enucleation)?

Holmium laser prostate enucleation (HoLEP) is a modern endoscopic BPH treatment based on enucleation of the prostate adenoma from its capsule using a holmium:YAG laser. It is particularly advantageous in large prostates greater than 80 grams.

Laser energy mechanically separates the prostate adenoma from the capsule and pushes it into the bladder; it is then fragmented and removed by morcellation. This technique was developed as an alternative to open prostatectomy.

Advantages of HoLEP include applicability independent of prostate size, low bleeding risk, short catheterisation time, shorter hospital stay and safe application in patients on anticoagulants. It has a learning curve and requires an experienced surgeon.

Symptoms

Prostate larger than 80 grams
BPH unresponsive to treatment
Patients on anticoagulants
Recurrent retention
Acute urinary retention
Haematuria
Bladder stone with BPH

Risk Factors

Advanced age
Severe comorbidity
Active infection
Large bladder diverticulum
Uncontrolled diabetes
Severe urethral stricture
Pelvic anomaly
