What is HoLEP (Holmium Laser Enucleation of Prostate)?

Holmium Laser Enucleation of the Prostate (HoLEP) is a minimally invasive endoscopic procedure using a holmium:YAG laser to enucleate prostatic adenoma along the surgical capsule plane, mimicking open simple prostatectomy through a transurethral approach.

The technique is considered size-independent and effective for prostates larger than 80-100 grams, where TURP becomes less effective. Postoperative morcellation removes the enucleated tissue. Compared to TURP, HoLEP offers lower transfusion rates, shorter catheterization, and durable functional outcomes.

Long-term data show excellent durability with low retreatment rates, making HoLEP a gold-standard option for BPH surgical management. Learning curve is steep, requiring experienced endourologists for optimal outcomes.

Symptoms

Lower urinary tract symptoms (LUTS)
Weak urinary stream
Urinary retention
Recurrent urinary tract infections

Failure of medical therapy
Recurrent retention
Bladder stones with BPH
Patients on anticoagulants (HoLEP advantage)
Need for tissue diagnosis
Younger patients seeking durable outcome
