What is Approach to the Frail HIV Patient?

Older and frail HIV patients present with functional limitations, polypharmacy, cognitive complaints, and social support difficulties. Geriatric assessment should be integrated with HIV care across both ambulatory and inpatient settings.

Careful monitoring is required for drug-drug interactions, fall risk, and malnutrition; frailty indices (e.g., Fried phenotype) and functional measures should be re-evaluated periodically.

The care plan must be multidisciplinary and patient-centered, involving HIV specialists, geriatricians, pharmacists, nutritionists, and social workers.
