What is HIV Comorbidity Monitoring?

Successful antiretroviral therapy has converted HIV into a chronic condition with non-AIDS comorbidities now driving morbidity. These include atherosclerotic cardiovascular disease, chronic kidney disease, low bone mineral density, neurocognitive impairment, malignancy, and metabolic syndrome.

Mechanisms include chronic immune activation, antiretroviral toxicities (tenofovir disoproxil for kidney and bone, protease inhibitors for lipids), persistent inflammation, and traditional risk factors at higher prevalence. Aging amplifies these risks.

Annual monitoring includes lipid panel, fasting glucose, eGFR with urine protein, DEXA scan from age 50, neurocognitive screening, age-appropriate cancer screening, and cardiovascular risk calculation with REPRIEVE pitavastatin recommendation when indicated.
