What is HIFU (High-Intensity Focused Ultrasound) — Prostate?

HIFU (High Intensity Focused Ultrasound) is a technique that creates fully controlled thermal necrosis by heating prostate tissue to 80-100°C with high-intensity ultrasound waves delivered through a transrectal probe.

It is applied as a treatment alternative in selected patients with localized low-to-intermediate risk prostate cancer and recurrent cases. It can be performed as whole-gland or focal approach.

Its advantages include less morbidity compared to invasive surgery, faster recovery, and generally better preservation of erection and continence. However, long-term oncological outcomes are not as mature as radical surgery.

Symptoms

Low-to-intermediate risk localized prostate cancer
Patients unsuitable for surgery
Local recurrence after radiotherapy
Indication for focal treatment
Quality-of-life-oriented preference
Patients with high surgical risk
Treatment requirement at advanced age

Risk Factors

Large prostates (difficulty for whole-gland HIFU)
Calcifications (decrease ultrasound penetration)
Previous radiotherapy (tissue effect)
Rectum pathologies
High-risk cancer subtypes
Multifocal high-volume disease
Metastatic disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Diagnosis of localized prostate cancer
  • High risk for surgery
  • If transition from active surveillance to treatment is being considered
  • Biochemical recurrence after radiotherapy
  • Patient-based focal treatment preference

Treatment Methods

01
Patient selection with multiparametric MRI and target biopsy
02
Transrectal HIFU application
03
Whole-gland or focal protocol
04
Post-HIFU catheterization
05
Follow-up with PSA and imaging
06
Salvage surgery or radiotherapy when needed
