What is Hepatitis D (Delta) Superinfection?

The hepatitis D virus (HDV) is a defective RNA virus that requires HBsAg for replication. For this reason it can only coexist with HBV.

Addition of HDV to a chronic HBV carrier (superinfection) leads to a much more severe clinical picture, rapid liver damage, early development of cirrhosis, and increased risk of hepatocellular carcinoma.

Diagnosis is made with anti-HDV and HDV RNA testing; treatment includes pegylated interferon and the newer agent bulevirtide.

Symptoms

Sudden deterioration in a patient with chronic HBV
Marked jaundice
Abdominal pain
Nausea and loss of appetite
Encephalopathy (in severe cases)
Rapidly progressing signs of cirrhosis
Severe fatigue

Risk Factors

Chronic HBV infection
Living in HBV-endemic areas
Intravenous drug use
Multiple sexual partners
HIV co-infection
Hemodialysis
Migration from HDV-endemic regions

When to See a Doctor?

  • Sudden ALT elevation in a chronic HBV patient
  • Clinical worsening and increasing jaundice
  • Screening of migrants from HDV-endemic areas
  • Signs of liver failure
  • As part of HCC screening
