What is Splenomegaly Differential Diagnosis in Hematology?

Splenomegaly indicates an enlarged spleen palpable below the costal margin or measured by imaging. Categories of cause include congestive splenomegaly from portal hypertension, infiltrative disease from leukemia, lymphoma, myeloproliferative neoplasms, or storage disorders, infections such as endocarditis and visceral leishmaniasis, and immunologic conditions such as autoimmune hemolysis and rheumatoid arthritis.

History and physical assessment focus on weight loss, night sweats, jaundice, alcohol use, travel exposure, and tender enlargement. Laboratory tests include complete blood count with differential and smear, liver and kidney function, viral hepatitis serology, peripheral blood flow cytometry, and lactate dehydrogenase.

Imaging with abdominal ultrasound or computed tomography defines size, focal lesions, and portal venous anatomy. Bone marrow examination, splenic biopsy, or splenectomy may be needed when peripheral testing is inconclusive in suspected lymphoma or storage disease.

Symptoms

Left upper quadrant fullness or pain
Early satiety from gastric compression
Cytopenias from hypersplenism
Night sweats and weight loss in lymphoma
Bruising and bleeding tendency

Risk Factors

Chronic liver disease with portal hypertension
Hematologic malignancy or myeloproliferative neoplasm
Travel to leishmaniasis or malaria areas
Storage disease such as Gaucher
Autoimmune hemolytic anemia
