What is Guided Implant Surgery in Dental Implantology?

Guided implant surgery represents a paradigm shift from traditional freehand placement to digitally planned, prosthetically-driven implant dentistry. The workflow integrates CBCT volumetric imaging with surface scans of teeth and soft tissue (intraoral scanner or model digitization), virtual diagnostic wax-up, and implant planning software to determine the ideal three-dimensional implant position based on the planned final restoration rather than available bone alone.

Surgical guides are classified by support: tooth-supported (most accurate, used in partially dentate patients), mucosa-supported (edentulous arches, requires anchor pins for stabilization), and bone-supported (rarely used today). Guides can be fully guided (drilling depth and final implant placement controlled by the guide via metal sleeves and implant-specific drill keys) or partially guided (only initial pilot drilling). Static guides are 3D printed; dynamic navigation uses real-time tracking without a physical guide.

Advantages include improved accuracy (mean deviation 1.0-1.5 mm at apex vs 2-3 mm freehand), reduced surgical time, possibility of flapless surgery (less postoperative discomfort and edema), facilitation of immediate loading with prefabricated provisionals, and predictability for full-arch rehabilitations (All-on-4/6). Limitations include cost, learning curve, accuracy degradation with mouth opening limitations, and dependency on accurate scan/CBCT registration. Complications include sleeve wear, deviation in soft bone, and overheating during continuous drilling without external irrigation.
