Gonorrhea

What is Gonorrhea?

Gonorrhea is caused by the Gram-negative diplococcus Neisseria gonorrhoeae and is largely transmitted sexually, infecting mucosal surfaces (urethra, cervix, rectum, pharynx, conjunctiva).

It is often asymptomatic in women; in men it typically presents with purulent urethral discharge and dysuria. Untreated, it can cause pelvic inflammatory disease, epididymitis, infertility and disseminated gonococcal infection.

Treatment relies on ceftriaxone with or without azithromycin given the rising resistance, with partner treatment and re-testing essential.

Symptoms

Purulent urethral discharge in men
Burning on urination
Vaginal discharge and pelvic pain in women
Rectal discharge and tenesmus
Pharyngitis and sore throat
Conjunctivitis
Disseminated infection: arthritis, rash

Risk Factors

Unprotected intercourse
Multiple sexual partners
New sexual partner
Other sexually transmitted infections
Sex work
Men who have sex with men
Untreated partner

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If purulent urethral discharge or dysuria appears
  • With abdominal or pelvic pain and fever
  • With rectal discharge and pain
  • If a partner has a positive diagnosis
  • Persistent sore throat with lymphadenopathy (pharyngitis)
