What is Gastropexy?

Gastropexy is a surgical procedure that fixes the stomach to the abdominal wall, diaphragm or surrounding structures because of abnormal stomach position, gastric volvulus or large hiatal hernias. The aim is to prevent stomach displacement or rotation.

Several techniques exist: anterior gastropexy (anterior wall of the stomach is fixed to the anterior abdominal wall), posterior gastropexy and tube gastropexy (in conjunction with percutaneous endoscopic gastrostomy, PEG). It can also be performed minimally invasively via laparoscopic approach.

Gastropexy is generally used as a definitive treatment or as an additional procedure in repair of large hiatal hernias. In acute treatment of gastric volvulus, it is preferred for fixation after reduction in non-necrotic situations. Short procedure time provides advantages for elderly and high-risk patients.

Symptoms

Acute or chronic gastric volvulus
Large type III or type IV paraesophageal hiatal hernia
Postprandial vomiting, bloating, dysphagia
Retrosternal pressure and pain
Respiratory distress (diaphragmatic elevation)
Weight loss and feeding difficulty
Recurrent aspiration pneumonia

Risk Factors

Advanced age and weak gastric ligaments
Diaphragm weakness and paraesophageal hernia
Previous upper GI surgery
Chronic increased intra-abdominal pressure
Congenital diaphragm anomalies
Gastric ligament laxity (gastrohepatic ligament)
Elderly patient with high surgical risk

When to See a Doctor?

  • Acute gastric volvulus signs (Borchardt triad)
  • Chronic bloating, postprandial pain and vomiting
  • Progressive symptoms in paraesophageal hernia
  • Recurrent aspiration pneumonia in the elderly
  • Gastric air seen in the chest on chest X-ray
  • Emergency surgical planning
