What is Functional Septorhinoplasty?

Functional septorhinoplasty integrates septoplasty (correction of deviated nasal septum) with rhinoplasty (modification of external nasal framework). Approaches: open (transcolumellar) for complex deformities, closed (endonasal) for limited reshaping. The procedure addresses internal nasal valve angle (10-15° normal), external nasal valve, septal deviation, turbinate hypertrophy, and dorsum/tip aesthetics.

Functional components: septoplasty with cartilage scoring/cross-hatching, spreader grafts (widen internal valve), alar batten grafts (support external valve), turbinoplasty (inferior turbinate reduction). Aesthetic components: dorsal hump reduction (preservation rhinoplasty popular - dorsal preservation, structural rhinoplasty for revision), tip refinement (tip suturing, lateral crural strut grafts), osteotomy (lateral, transverse, medial).

Outcomes: NOSE score improvement >50%, ROE (Rhinoplasty Outcome Evaluation) high satisfaction in 85-90%. Revision rate 5-15% (functional/aesthetic). Recovery: splints 7 days, edema resolves 6-12 months, final result 12-18 months. Complications: bleeding, infection, septal perforation, saddle nose, persistent deviation.
