What is Frontal Sinusitis?

The frontal sinuses lie between the posterior wall of the forehead and the brain; this proximity allows infection to spread easily into the brain or orbits.

Complications of frontal sinusitis include epidural abscess, subdural empyema, meningitis and orbital complications. Therefore, imaging and specialist evaluation are mandatory in cases unresponsive to therapy.

Acute frontal sinusitis usually starts with ostial obstruction caused by viral upper respiratory infection or nasal polyps.

Symptoms

Severe pain in the middle of the forehead or above the eyes
Headache more pronounced in the morning
Tenderness of the forehead skin and pain on touch
Nasal discharge and obstruction
Fever and malaise
Rare: eyelid swelling or visual changes

Risk Factors

Nasal polyps and septal deviation
Allergic rhinitis
Barotrauma (aviation, diving)
Immunodeficiency
Recurrent acute sinusitis episodes
Anatomical variants affecting the nasofrontal duct

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If forehead pain persists for more than 7 days
  • If there is periorbital swelling, redness or proptosis
  • Combination of fever and severe headache
  • Neck stiffness or altered consciousness
  • If there is no response to antibiotic therapy
