What is Fournier Gangrene?

Fournier gangrene is a rapidly progressive form of necrotizing fasciitis of the perineum, genitals, and perianal region caused by a mixture of aerobic and anaerobic bacteria. Its mortality is high if early diagnosis and radical debridement are not performed.

It is most commonly seen in men; diabetes, obesity, immunosuppression, and urogenital-colorectal infections are the main predisposing factors.

Locally mild-appearing infection at onset may rapidly progress to systemic sepsis; for this reason, an aggressive surgical approach when in doubt is life-saving.

Symptoms

Severe pain in the perineum, scrotum, or perianal region
Erythema, edema, and crepitus
Foul-smelling discharge and necrosis
High fever and chills
Tachycardia and hypotension
Altered consciousness
Skin color change (purplish, blackening)

Risk Factors

Diabetes
Obesity
Alcohol use
Immunosuppression (HIV, chemotherapy, steroid)
Chronic kidney failure
Perianal abscess, fistula, urethral stricture
History of trauma and surgical intervention

When to See a Doctor?

  • Rapidly increasing pain and swelling in the genital region
  • Change in skin color
  • High fever and signs of sepsis
  • Perineal infection in diabetics
  • Foul-smelling discharge and crepitus
