What is Fever and Rash Algorithm in Adults?

Fever with rash refers to the simultaneous presence of body temperature ≥38°C and any cutaneous eruption in an adult, requiring rapid risk-stratification to identify life-threatening infectious, drug-induced or autoimmune causes.

A structured algorithm screens for danger signs (hypotension, mucous membrane involvement, purpura, blistering, lymphadenopathy, recent travel or exposure, drug history) and uses morphology of the rash (purpuric, maculopapular, vesicular, pustular, erythrodermic) to focus differential diagnosis.

High-priority causes include meningococcemia, Rocky Mountain spotted fever and other rickettsioses, Lyme/erythema migrans, viral exanthems (measles, EBV, HIV), Stevens–Johnson/toxic epidermal necrolysis, DRESS, drug-induced AGEP, vasculitides and adult-onset Still disease.

Symptoms

Acute fever and skin eruption
Petechiae or palpable purpura
Mucosal involvement
Skin pain or tenderness
Hypotension or tachycardia
Lymphadenopathy
Recent new drug or travel exposure

Risk Factors

Recent international travel
New medication in last 8 weeks
Tick or insect exposure
Sick contacts with similar illness
Sexually transmitted infection risk
Immunosuppression or pregnancy
Outbreak setting

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Fever with petechial rash
  • Mucosal blistering or sloughing
  • Hypotension or altered mental status
  • Travel from endemic area with fever and rash
  • New rash days to weeks after starting medication
  • Rapidly progressive skin pain
  • Pregnancy with rash and fever
