What is ESBL-Producing Bacterial Infections?

Extended-spectrum beta-lactamases (ESBLs) are enzymes that hydrolyze penicillins and third-generation cephalosporins. They are most commonly seen in E. coli and Klebsiella pneumoniae strains.

ESBL producers are increasingly identified in community-acquired urinary tract infections as well; rectogenital colonization plays a role in transmission and recurrence.

Early diagnosis and appropriate antibiotic selection reduce complications, length of stay, and mortality.

Symptoms

Recurrent urinary tract infection
Fever and flank pain
Signs of systemic infection
Bacteremia and sepsis
Lack of clinical response to therapy

Risk Factors

Broad-spectrum antibiotic use
Recent hospitalization
Urinary catheter
Older age and diabetes
Travel history (high-prevalence regions)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When a UTI is unresponsive to standard antibiotics
  • When sepsis develops
  • With a history of recurrent pyelonephritis
  • In febrile UTI in older or immunosuppressed patients
