What is Erysipelas?

Erysipelas is an acute bacterial infection involving the superficial dermis and superficial lymphatics, characterized by a sharply demarcated bright red plaque. It is most often caused by Group A beta-hemolytic streptococci.

It most commonly occurs on the legs and face. The sudden onset of the lesion, sharp border, and increased warmth aid diagnosis.

It responds rapidly to treatment; however, it has a tendency to recur and can lead to chronic lymphedema due to lymphatic damage.

Symptoms

Bright red, warm, tender plaques
Sharp, raised border
Sudden fever and chills
Malaise and headache
Regional lymphadenopathy
Bullous lesions (severe cases)
Recurrent attacks

Risk Factors

Chronic venous insufficiency of the leg
Lymphedema
Foot fungal infections (entry portal)
Obesity
Diabetes
Any condition disrupting the skin barrier
Previous erysipelas attacks

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If sudden painful redness occurs on the leg or face
  • If fever and chills are present
  • If the lesion is rapidly expanding and spreading
  • For a new attack in a patient with prior similar episodes
  • If systemic signs (low blood pressure, altered consciousness) develop (emergency)
