What is Epigastric Hernia?

Epigastric hernia is the protrusion of preperitoneal fat or rarely omentum through a defect in the linea alba between the xiphoid bone and the umbilicus. It is usually small and single-defect, mostly an omental hernia not containing bowel.

It is three times more common in men than in women. It is mostly asymptomatic or presents with mild localized pain; however, entrapment of fatty tissue may cause chronic pain in the epigastric region.

Primary suture repair is sufficient for small defects, while large defects and recurrences require mesh placement.

Symptoms

Small soft swelling above the umbilicus
Protrusion that becomes prominent with coughing, straining, or standing
Chronic pain in the epigastric region (omental entrapment)
Sudden severe pain and tenderness in strangulation

Risk Factors

Male gender and middle age
Obesity and increased intra-abdominal pressure
Chronic cough and constipation
Heavy physical workload
Connective tissue weakness

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Growing swelling above the umbilicus
  • Tenderness in the swelling and inability to reduce it (strangulation)
  • Epigastric pain affecting daily activity
  • Growing or painful epigastric mass in children
