What is Epididymectomy?

Epididymectomy is the surgical removal of the epididymis, the structure located on the posterior and superior aspect of the testis. It is performed for chronic epididymitis, recurrent epididymitis, large epididymal cysts (spermatocele) and rarely for epididymal tumors.

The procedure is performed through a scrotal incision, with the epididymis carefully separated from the testis and removed while preserving the testis and its blood supply. The operation may be unilateral or bilateral.

Bilateral epididymectomy results in infertility, so the decision must be made carefully in patients who desire children. Postoperative pain reduction is reported to be in the range of 50-90%.

Symptoms

Chronic epididymal pain (longer than 3 months)
Recurrent episodes of epididymitis
Enlarging epididymal cyst (spermatocele)
Scrotal pain and tenderness
Induration or mass in the epididymis
Infection unresponsive to antibiotic therapy

Risk Factors

Post-vasectomy pain syndrome
Recurrent urinary tract infection
Tuberculous epididymitis
Amiodarone use (drug-induced epididymitis)
Previous inguinal surgery
Urinary reflux (in children)
