What is Zenker's Diverticulum?

Zenker's diverticulum is a pseudodiverticulum formed by posterior herniation of pharyngeal mucosa through Killian's triangle, the weak area between the thyropharyngeus and inferior pharyngeal constrictor muscles.

It is most often seen in men over 60 years of age and is driven by cricopharyngeal muscle dysfunction with inadequate relaxation. Food retained inside the pouch produces halitosis and predisposes to aspiration.

Treatment is surgical, either by an endoscopic approach or by an open cervical procedure that combines cricopharyngeal myotomy with diverticulectomy or diverticulopexy.

Symptoms

Dysphagia for solids and liquids
Regurgitation of undigested food
Halitosis
Gurgling sounds in the neck
Chronic cough and aspiration
Unintentional weight loss

Risk Factors

Advanced age (over 60 years)
Male sex
Cricopharyngeal muscle spasm
Gastro-oesophageal reflux
Cervical osteophytes
