What is Subglottic Stenosis?

Subglottic stenosis is a narrowing of the airway extending from just below the glottis to the upper trachea. The congenital form is rare, while acquired stenosis most often follows prolonged endotracheal intubation that produces mucosal injury and scar formation.

According to the Myer-Cotton classification the lesion is graded into four levels: grade I (0-50% obstruction) causes mild symptoms, whereas grade IV (99-100% obstruction) requires tracheotomy.

Treatment is tailored to severity and ranges from endoscopic balloon dilatation and laser resection to open laryngotracheal reconstruction.

Symptoms

Exertional dyspnoea
Inspiratory stridor
Recurrent croup-like episodes in children
Progressive shortness of breath on exertion
Hoarseness

Risk Factors

Prolonged endotracheal intubation
Previous tracheotomy
Gastro-oesophageal reflux
Autoimmune disease (e.g. granulomatosis with polyangiitis)
History of head-and-neck radiotherapy
Prematurity
