What is Endoscopic Optic Nerve Decompression?

Endoscopic optic nerve decompression is performed by accessing the medial wall of the optic canal through the sphenoid sinus and posterior ethmoid; the bony portion of the canal is thinned with a diamond burr and removed to relieve pressure on the optic nerve.

Main indications include traumatic optic neuropathy, fibrous dysplasia involving the optic canal, and compressive optic neuropathy in thyroid orbitopathy. Selected cases may also include optic nerve sheath meningioma and idiopathic intracranial hypertension.

Avoidance of open craniotomy results in low morbidity and short recovery; outcomes depend strongly on prompt intervention, often within 7 days for traumatic cases, before irreversible optic atrophy develops.

Sudden or progressive vision loss
Color vision loss
Relative afferent pupillary defect (RAPD)
Visual field defect
Optic disc edema or pallor
Post-traumatic visual decline
