What is Endolymphatic Hydrops MRI Diagnosis?

Endolymphatic hydrops is the pathological enlargement of the endolymphatic compartment caused by absorption failure or overproduction of endolymph. It underlies Meniere disease and may complicate inner ear trauma, autoimmune conditions, and migraine.

Magnetic resonance imaging at 3 Tesla using delayed three-dimensional fluid-attenuated inversion recovery sequences four hours after intravenous or intratympanic gadolinium injection allows the contrast agent to permeate perilymph while remaining excluded from endolymph. The endolymphatic space appears as a non-enhancing area within the contrast-filled perilymph, enabling visualization and grading of hydrops in cochlea, vestibule, and semicircular canals.

Standardized grading systems (Nakashima, Bernaerts) classify hydrops as none, mild, or significant. Imaging findings correlate with audiovestibular symptoms, support diagnosis of definite Meniere disease, and inform treatment decisions including intratympanic steroid or gentamicin therapy. The test is increasingly available in tertiary otology centers.
