What is Dumping Syndrome?

Dumping syndrome is a postprandial vasomotor and gastrointestinal symptom complex that occurs after gastric surgeries (gastrectomy, gastric bypass, pyloroplasty) due to rapid emptying of stomach contents into the small intestine.

Early dumping occurs 15-30 minutes after eating and arises when hyperosmolar contents draw fluid into the small intestine. Late dumping occurs 1-3 hours later and is characterized by reactive hypoglycemia.

Diagnosis is generally made by clinical evaluation; oral glucose tolerance test and gastric emptying studies are supportive. Treatment begins with dietary changes (small frequent meals, carbohydrate restriction), followed by medical therapy (acarbose, octreotide), and rarely surgical revision.

Symptoms

Postprandial palpitations and sweating
Abdominal pain and cramping
Diarrhea and nausea
Dizziness and weakness
Reactive hypoglycemia
Tachycardia
Feeling of fainting

Risk Factors

History of gastrectomy
Roux-en-Y gastric bypass
Pyloroplasty
Vagotomy
High carbohydrate intake
Combined intake of liquids and solids
Fast eating
