Diaphragmatic Hernia Repair

Surgical correction of a defect in the diaphragm — congenital (Bochdalek, Morgagni) or acquired hiatal or traumatic — through which abdominal contents protrude into the chest.

What is Diaphragmatic Hernia Repair?

Diaphragmatic hernia is a defect in the diaphragm through which abdominal organs herniate into the thorax; the most common types are sliding hiatal hernia, paraesophageal hernia, traumatic diaphragmatic injury, and congenital Bochdalek and Morgagni hernias.

In adults, surgery is most commonly performed for symptomatic paraesophageal or large sliding hernias with severe gastroesophageal reflux disease, and traumatic ruptures require emergency repair.

Repair is most often performed laparoscopically with hiatal closure and a fundoplication for hiatal hernias, while large defects may require mesh reinforcement; congenital defects in newborns are repaired by pediatric surgical teams in specialized centers.

Symptoms

Severe and persistent heartburn or regurgitation in hiatal hernia
Chest pain, dysphagia, and postprandial fullness in paraesophageal hernia
Chronic anemia from chronic blood loss (Cameron lesions)
Sudden severe pain or vomiting suggesting incarceration or strangulation
Respiratory distress in newborns with congenital diaphragmatic hernia

Risk Factors

Advanced age and obesity
Pregnancy and chronically increased intra-abdominal pressure
Trauma — blunt or penetrating thoracoabdominal injury
Congenital developmental defects
Connective tissue disorders

When to See a Doctor?

  • Severe symptomatic gastroesophageal reflux disease unresponsive to medical therapy
  • Dysphagia, chest pain, or postprandial fullness suggesting paraesophageal hernia
  • Sudden severe pain, vomiting, or bleeding — emergency evaluation
  • Suspected diaphragmatic injury in chest or abdominal trauma
