Cryoglobulinemic Vasculitis

What is Cryoglobulinemic Vasculitis?

Cryoglobulinemic vasculitis is a systemic vasculitis triggered by deposition of cold-precipitable immunoglobulins (cryoglobulins) in small and medium vessels. Mixed (type II) and polyclonal (type III) cryoglobulinemias are the most common vasculitic forms.

Hepatitis C virus accounts for 70 to 90 percent of mixed cryoglobulinemia cases; other infections, autoimmune diseases, and lymphoproliferative disorders may also be responsible.

The classic Meltzer triad — palpable purpura, arthralgia, and weakness — is highly suggestive. Renal involvement typically presents as membranoproliferative glomerulonephritis. Treatment focuses on the underlying cause, especially HCV eradication.

Symptoms

Palpable purpura, particularly on the lower extremities
Arthralgia
Raynaud phenomenon
Peripheral neuropathy
Renal involvement — proteinuria and hematuria
Generalized malaise and fatigue
Cutaneous ulcerations
