What is Component Separation Hernia Repair?

Component Separation Technique (CST) is an abdominal wall reconstruction approach that mobilizes muscle and fascial layers to achieve tension-free midline closure in large or complex ventral hernias, particularly when primary closure is impossible.

Anterior component separation (Ramirez technique) releases the external oblique aponeurosis, while posterior component separation with transversus abdominis release (TAR) provides greater medial advancement and creates a large retromuscular plane for mesh placement.

Indications include incisional hernias greater than 10 cm in width, recurrent hernias, loss of domain, and complex defects. Combined with sublay mesh reinforcement, CST achieves durable repair with reduced recurrence rates.
