Colorectal Polypectomy

What is Colorectal Polypectomy?

Colorectal polypectomy is the endoscopic removal of adenomatous, serrated, or hyperplastic polyps during colonoscopy and is the cornerstone of colorectal cancer prevention.

Different techniques are selected based on polyp size, morphology, and location — cold snare for small (≤5 mm) polyps, hot snare and clip placement for medium-sized polyps, endoscopic mucosal resection (EMR) for sessile polyps larger than 20 mm, and endoscopic submucosal dissection (ESD) for selected sessile or laterally spreading lesions.

All resected polyps are sent for histopathological examination, and post-polypectomy follow-up is determined by the number, size, and pathology of the lesions.

Symptoms

Most polyps are asymptomatic and detected at screening colonoscopy
Painless rectal bleeding or hematochezia
Iron deficiency anemia from chronic occult blood loss
Change in bowel habits with large polyps
Mucous discharge with rectal villous adenomas

Risk Factors

Age over 50
Family history of colorectal cancer or polyposis syndromes
Lynch syndrome and familial adenomatous polyposis
Inflammatory bowel disease
Obesity, smoking, and high consumption of red and processed meat

  • Iron deficiency anemia of unknown cause
  • Family history of colorectal polyps or cancer

Treatment Methods

Bowel preparation with PEG-based or low-volume preparations and a clear-liquid diet

Cold snare polypectomy for ≤5 mm polyps

Hot snare polypectomy and clip placement for 6 to 19 mm polyps

Endoscopic mucosal resection (EMR) for sessile or pedunculated polyps ≥20 mm

Endoscopic submucosal dissection (ESD) for selected non-pedunculated polyps and follow-up colonoscopy according to histology and number of polyps
