What is Chronic Sinusitis?

Chronic sinusitis is persistent inflammation of the sinus mucosa exceeding 12 weeks. It is divided into two main subtypes based on polyp presence: chronic sinusitis with nasal polyps (CRSwNP) and without nasal polyps (CRSsNP).

In its pathophysiology, impaired mucociliary clearance, ostial obstruction and biofilm formation play a central role. Underlying allergy, asthma, immune deficiency or anatomical variants may complicate the condition.

If untreated it can lay the groundwork for serious complications such as orbital cellulitis, meningitis or osteomyelitis.

Symptoms

Nasal congestion exceeding 12 weeks
Anterior or posterior nasal discharge (postnasal drip)
Pressure and fullness sensation on the face
Decrease or loss of sense of smell
Chronic cough (especially at night)
Headache and fatigue

Risk Factors

Untreated acute sinusitis episodes
Allergic rhinitis or asthma
Nasal polyps and septal deviation
Smoking and air pollution exposure
Immune deficiency (primary or secondary)
Aspirin/NSAID hypersensitivity (Samter's triad)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When nasal symptoms exceed 12 weeks
  • When antibiotic treatment fails to provide a response
  • When sense of smell is completely lost
  • When swelling around the eyes or double vision develops
  • When high fever or severe headache is added
