What is Chronic Brucellosis?

Chronic brucellosis is the form of Brucella infection in which symptoms persist for months or years due to inadequate treatment or relapse, with focal involvement in various organs. It often presents with osteoarticular, cardiovascular, and neurological manifestations.

The most commonly involved sites are the sacroiliac joints and the vertebral column; osteomyelitis, discitis, and paravertebral abscesses may develop. Cardiac involvement may present as bacterial endocarditis, while central nervous system involvement manifests as neurobrucellosis.

Diagnosis is based on clinical history, serology, and culture. Treatment requires prolonged combination antibiotic regimens, with surgical drainage and local intervention indicated in selected cases.

Symptoms

Persistent fatigue and weakness
Undulant fever and night sweats
Sacroiliac and vertebral pain
Myalgia and depressed mood
Joint swelling and restricted motion
Findings of endocarditis or meningitis
Hepatosplenomegaly

Risk Factors

Inadequate or inappropriate treatment of acute brucellosis
Continued consumption of raw milk and fresh cheese
Animal husbandry and veterinary work
Immunosuppressive states
Prolonged exposure to the source
Delayed diagnosis
