What is Chikungunya?

Chikungunya is an arbovirus infection caused by an alphavirus from the Togaviridae family, transmitted by infected Aedes aegypti and A. albopictus mosquitoes. The name means 'bent person' in the Makonde language and was given in reference to severe joint pain.

The disease begins with sudden onset high fever and severe symmetric polyarthralgia after a 2-12 day incubation period. Wrists, ankles, and finger joints are particularly affected. Rash, headache, and muscle aches may accompany.

The acute phase generally resolves in 1-2 weeks; however, in a significant proportion of patients, joint pain may last weeks to months, even years. It is not fatal but seriously affects quality of life.

Symptoms

Sudden onset high fever (39-40°C)
Severe symmetric joint pain and swelling
Hand, wrist, ankle, and knee involvement
Maculopapular rash
Muscle pain and headache
Weakness and loss of appetite
Chronic joint pain (months to years)

Risk Factors

Travel to outbreak areas (Africa, Asia, Latin America)
Aedes mosquito population
Rainy season
Standing water sources
Advanced age (for chronic form)
Pre-existing joint disease
Failure to take personal protection measures

  • Fever and joint pain after returning from endemic area
  • Severe joint pain and movement limitation
  • Joint complaints lasting weeks to months
  • High fever and rash
  • Development of chronic arthritis
