Cecal Volvulus

An emergency in which the cecum twists around its mesentery, leading to bowel obstruction and ischemia.

What is Cecal Volvulus?

Cecal volvulus is a closed-loop obstruction occurring with rotation around the mesenteric axis on the basis of inadequate retroperitoneal fixation of the cecum and ascending colon (mobile cecum).

It is less common than sigmoid volvulus but has higher mortality because it does not respond adequately to endoscopic decompression and ischemia develops rapidly.

On plain radiograph, the 'coffee bean' sign, an image of an enlarged cecum located in the left upper quadrant, leads to the diagnosis; it is confirmed by CT.

Symptoms

Sudden-onset severe abdominal pain
Marked abdominal distension
Vomiting
Cessation of gas and stool passage
Tympanic area in the left upper quadrant
Tachycardia and dehydration
Peritonitis findings in advanced cases

Risk Factors

Mobile cecum (congenital)
Previous abdominal surgery
Chronic constipation
High-fiber diet and fermented foods
Advanced age
Long-term bed rest
Pregnancy (rare)

  • Bilious vomiting and absence of gas-stool passage

  • Defense and rebound in the abdomen
