What is Carcinoid Tumor Resection Surgery?

Carcinoid tumour is a low-grade neuroendocrine tumour (NET); the most common locations are the small intestine, appendix and lung.

Surgical excision is the only curative treatment. The mainstays of treatment are appendectomy in appendix tumours <1 cm, right hemicolectomy in tumours >2 cm and segmental resection in small-intestinal carcinoids.

Symptoms

Asymptomatic incidental finding (most common)
Carcinoid syndrome (flushing, diarrhoea, bronchospasm; in liver metastasis)
Right lower quadrant abdominal pain (appendiceal carcinoid)
Recurrent abdominal pain (mesenteric ischaemia, small-bowel obstruction)
Cough, haemoptysis (bronchial carcinoid)

Risk Factors

MEN1 syndrome (multiple endocrine neoplasia)
Neurofibromatosis type 1
von Hippel-Lindau syndrome
Family history (rare)
Female gender (slight predominance)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Suspected carcinoid syndrome (flushing, diarrhoea)
  • Carcinoid foci on appendectomy pathology
  • Submucosal mass on small-bowel imaging
  • Bronchial carcinoid suspicion (cough, haemoptysis)

Treatment Methods

01
Appendectomy (in tumour <1 cm)
02
Right hemicolectomy (>2 cm appendiceal carcinoid; mesoappendix involvement)
03
Segmental small-bowel resection + lymph node dissection (in small-bowel carcinoids)
04
Anatomic lung resection - lobectomy (bronchial carcinoid)
05
Mesenteric mass excision (in mesenteric fibrosis)
06
Hepatic debulking (in liver metastasis; symptom palliation)
07
Octreotide (preoperative carcinoid crisis prophylaxis)
08
Postoperative chromogranin A and 5-HIAA monitoring
09
Ga-68 DOTATATE PET (recurrence imaging)
