What is BCMA-Targeted CAR-T Cell Therapy for Multiple Myeloma?

Patient T cells are collected through leukapheresis, genetically modified to express anti-BCMA chimeric antigen receptors and infused back to the patient.

BCMA expression on plasma cells provides specific tumor target with limited expression on other tissues.

Approved indications include heavily pretreated relapsed/refractory multiple myeloma after multiple prior therapies.

Lymphodepleting chemotherapy with fludarabine and cyclophosphamide precedes CAR-T cell infusion to optimize engraftment.

Cells expand and persist in vivo providing prolonged antitumor activity though duration varies between products and patients.

Symptoms

Cytokine release syndrome occurs commonly with fever, hypotension, hypoxia and organ dysfunction requiring tocilizumab and corticosteroids.
Immune effector cell-associated neurotoxicity syndrome (ICANS) presents with confusion, encephalopathy, seizures and rarely cerebral edema.
Cytopenias including prolonged neutropenia, anemia and thrombocytopenia require supportive care and growth factor support.
Infections particularly opportunistic infections occur due to lymphodepletion and B-cell aplasia requiring antimicrobial prophylaxis.
Hypogammaglobulinemia commonly develops requiring immunoglobulin replacement therapy.

Risk Factors

High disease burden at apheresis time may increase cytokine release syndrome severity.
Performance status and organ function affect tolerability of intensive treatment.
Prior allogeneic stem cell transplant or extensive prior therapy affects T-cell quality and engraftment.
Active infection requires evaluation and treatment before CAR-T cell administration.
Cardiac, pulmonary or renal dysfunction limits ability to tolerate cytokine release syndrome.

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Fever, hypotension, dyspnea or other systemic symptoms during CAR-T cell therapy require immediate hospital evaluation.
  • Confusion, headache, weakness or seizures require urgent neurologic evaluation for ICANS.
  • Bleeding, easy bruising or signs of infection require evaluation for cytopenias and possible transfusion or antimicrobial therapy.
  • Persistent fatigue or new symptoms after discharge require oncologic evaluation given prolonged immunosuppression.
  • Severe complications require intensive care management with multidisciplinary team.
