What is Candidemia and Invasive Candidiasis?

Candidemia is a severe fungal infection that develops when Candida species reach the bloodstream, generally seen in hospitalized immunocompromised patients.

Central venous catheter use, prolonged broad-spectrum antibiotics, total parenteral nutrition, ICU stay, and neutropenia are leading risk factors.

Clinically, fever, signs of sepsis, and organ involvement (endophthalmitis, endocarditis) are observed. Early diagnosis and appropriate antifungal therapy such as echinocandins reduce mortality.

Symptoms

Fever and chills (unresponsive to antibiotics)
Sepsis and hypotension
Redness at catheter entry site
Skin rashes
Blurred vision (endophthalmitis)
Heart murmur (endocarditis)
Organ failures

Risk Factors

ICU stay
Central venous catheter
Prolonged antibiotic use
Immunosuppressive therapy
Surgery and abdominal infection
Hemodialysis
Neutropenia and transplantation

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Antibiotic-unresponsive fever in an ICU patient
  • Fever and chills in a patient with a catheter
  • When blood culture grows Candida
  • When new visual disturbances develop
  • Hemodynamic deterioration

Treatment Methods

01
Echinocandin (caspofungin, micafungin) as first choice
02
Fluconazole based on susceptibility
03
Catheter removal when possible
04
Eye and cardiac evaluation
05
Susceptibility testing and dose optimization
06
Long-term therapy (at least 14 days after culture negativity)
