What is Cancer-Related Fatigue: Management?

Cancer-related fatigue is a persistent, distressing sense of physical, emotional, or cognitive tiredness related to cancer or its treatment, disproportionate to recent activity and not relieved by rest. It affects up to ninety percent of patients during chemotherapy and radiotherapy and persists in survivors for months to years after treatment.

Causes are multifactorial: anemia, hypothyroidism, hypogonadism, deconditioning, sleep disturbance, depression, anxiety, pain, malnutrition, and the inflammatory effects of cancer and its therapies. Screening at every oncology visit using a 0 to 10 scale is recommended; scores of 4 or higher trigger a structured evaluation for treatable contributors.

The strongest evidence for treatment is for graded aerobic and resistance exercise, which consistently outperforms pharmacologic options. Cognitive behavioral therapy, mindfulness-based stress reduction, and yoga also reduce fatigue. Pharmacotherapy with methylphenidate, modafinil, or ginseng has modest, mixed evidence and is reserved for selected patients. Treating anemia, sleep disorders, depression, and pain often produces the largest gains.

Risk Factors

Active chemotherapy or radiotherapy
Anemia, hypothyroidism, or hypogonadism
Untreated pain or depression
Deconditioning and physical inactivity
Polypharmacy with sedating drugs

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent fatigue scoring 4 or higher on a 0 to 10 scale
  • Fatigue that interferes with work, family, or self-care
  • New onset of marked tiredness during cancer survivorship
  • Fatigue accompanied by depression or hopelessness
