What is Reading the Cancer Pathology Report?

A complete pathology report includes specimen description, gross findings, microscopic diagnosis, tumor type and subtype, grade, stage information including TNM elements, surgical margin status, lymphovascular and perineural invasion, and ancillary studies such as immunohistochemistry, flow cytometry, and molecular testing.

Tumor grade reflects differentiation and is reported as well, moderately, or poorly differentiated for many carcinomas and uses scoring systems such as Gleason for prostate, Nottingham for breast, and Fuhrman for renal cell. Grade complements stage as an independent prognostic variable and influences adjuvant therapy decisions.

Molecular findings increasingly drive therapy selection. Estrogen, progesterone, and HER2 receptors guide breast cancer therapy. EGFR, ALK, ROS1, BRAF, and PD-L1 in lung cancer and KRAS, BRAF, and microsatellite instability in colorectal cancer support targeted and immune-based regimens. Reports use synoptic templates to ensure completeness.
