What is Cancer Metastasis Biology?

Metastasis is the process by which cancer cells spread from a primary tumor to distant organs and form secondary tumors. It involves a metastatic cascade including local invasion, intravasation into vessels, survival in circulation, extravasation, and adaptation to a foreign microenvironment.

Mechanisms include epithelial-mesenchymal transition, stromal interactions, angiogenesis, immune evasion, and organ-specific seeding (the 'seed and soil' hypothesis). Genomic and epigenomic changes, microenvironmental factors, and circulating tumor cells contribute to metastatic competence.

Most cancer-related deaths are due to metastatic disease. Therapies target tumor-cell dependencies, the microenvironment, immune evasion, and organ-specific homing. Research areas include circulating tumor cells, dormant cells, organotropism, and tumor heterogeneity.

Symptoms

Site-specific symptoms reflecting metastatic location
Bone pain or fractures from skeletal metastases
Cough or dyspnea from pulmonary metastases
Headache or neurological deficits from brain metastases
Constitutional symptoms (weight loss, fatigue)

Advanced primary tumor with high invasive potential
