What is Management of Cancer Bone Metastases?

Bone metastases are common in advanced breast, prostate, lung, kidney, and thyroid cancers and disrupt the balance between osteoblastic bone formation and osteoclastic resorption. Patients experience persistent localized pain, pathologic fractures, hypercalcemia, and spinal cord compression that together are called skeletal-related events.

Diagnostic work-up combines magnetic resonance imaging, computed tomography, bone scintigraphy, and selective positron emission tomography to map disease extent and assess fracture risk using validated scoring such as Mirels and the spinal instability neoplastic score. Biopsy may be needed when imaging is ambiguous or when histology directs molecular therapy.

Treatment integrates bone-modifying agents with denosumab or zoledronic acid, palliative external beam radiotherapy or radiopharmaceuticals such as radium-223 in prostate cancer, prophylactic or therapeutic surgical stabilization, and ongoing systemic anticancer therapy. Pain management, rehabilitation, and dental assessment for osteonecrosis prevention complete the program.

Symptoms

Persistent localized bone pain that worsens at night
Pathologic fracture with minor trauma
Spinal pain with neurologic deficits
Hypercalcemia with confusion or constipation
Reduced mobility and weight-bearing tolerance

Risk Factors

Metastatic breast or prostate carcinoma
Multiple myeloma or renal cell carcinoma
High volume axial skeletal disease
Prior osteoporosis or low bone density
Long-duration corticosteroid exposure

  • When bone pain becomes constant or nocturnal
  • When sudden severe spinal pain appears
  • When neurologic symptoms suggest cord compression
  • When pathologic fracture occurs
