What is Blepharitis?

Anterior blepharitis: staphylococcal or seborrheic inflammation of the lash line.

Posterior blepharitis: meibomian gland dysfunction (MGD) with altered oil secretion.

Mixed blepharitis is most common and coexists with dry eye.

Demodex infestation (D. folliculorum, D. brevis) may contribute to chronic or refractory cases.

Symptoms

Burning, itching, or foreign-body sensation
Crusting, scaling, or matter along the eyelashes
Red, swollen, or greasy eyelid margins
Styes and chalazia (recurrent)
Fluctuating blurred vision from tear film instability
Eye fatigue, light sensitivity, and lash loss/misdirection

Risk Factors

Rosacea and seborrheic dermatitis
Dry eye disease and meibomian gland dysfunction
Age over 50 years
Contact lens wear
Atopic disease (allergy, eczema)
Demodex infestation, especially in elderly

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Chronic redness, burning, or crusting despite home hygiene
  • Recurrent styes or chalazia
  • Eyelid mass, ulceration, or lash loss (rule out malignancy)

Treatment Methods

Daily lid hygiene: warm compresses (5–10 min) followed by lid scrubs with diluted baby shampoo or commercial lid cleansers

Topical antibiotic ointment (erythromycin, bacitracin) for staphylococcal anterior blepharitis

Oral doxycycline or azithromycin for moderate-severe posterior blepharitis/rosacea

Tea tree oil or ivermectin for Demodex infestation

Artificial tears and omega-3 supplementation for associated dry eye
