What is Blastic Plasmacytoid Dendritic Cell Neoplasm?

A rare aggressive myeloid neoplasm derived from precursors of plasmacytoid dendritic cells

Defined by characteristic immunophenotype expressing CD4, CD56, CD123, TCL1, and CD303

Recurrent involvement of skin, bone marrow, peripheral blood, and lymph nodes is typical

Genomic studies reveal mutations in TET2, ASXL1, ZRSR2, and chromosomal abnormalities of 6, 9, and 13

Median survival historically below 14 months despite intensive chemotherapy approaches

Symptoms

Bruise-like or nodular skin lesions of varying size and color across the body
Cytopenias from bone marrow infiltration causing fatigue, infections, and bleeding
Generalized lymphadenopathy and splenomegaly in advanced disease
Constitutional symptoms including fever, weight loss, and night sweats
Central nervous system involvement with neurologic deficits in selected cases

Risk Factors

Older male predominance with median age of 65 years at diagnosis
No clear environmental or hereditary risk factors have been identified
Possible association with prior or concurrent myelodysplastic syndrome
Recurrent somatic mutations in epigenetic regulators
Pediatric cases are rare but reported with similar biology

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Unexplained skin nodules, bruise-like lesions, or violaceous plaques requiring biopsy
  • Cytopenias accompanied by skin findings or lymphadenopathy
  • Bone marrow examination consistent with aggressive blast infiltrate
  • Suspicion of central nervous system involvement with focal neurologic signs
  • Need for urgent referral to a hematology center experienced in BPDCN
