What is Bladder Stone?

Bladder stones are concretions formed within the bladder, most often in adults as a secondary phenomenon to outlet obstruction and incomplete emptying.

Struvite, calcium oxalate, and uric acid compositions are common; chronic catheters favor struvite and infection stones.

Primary (idiopathic) endemic bladder stones in children remain prevalent in some developing regions.

Diagnosis is by KUB ultrasound, low-dose CT, or cystoscopy; all bladder stones warrant evaluation of outlet obstruction.

Symptoms

Intermittent lower-abdominal or suprapubic pain
Urinary frequency, urgency, and dysuria
Gross or microscopic hematuria, often at end of void
Intermittent urinary stream that stops and starts
Recurrent urinary tract infections
Acute urinary retention when a stone impacts the bladder neck

Risk Factors

Bladder outlet obstruction from BPH or urethral stricture
Neurogenic bladder (spinal cord injury, spina bifida, MS)
Chronic indwelling catheter or suprapubic catheter
Bladder diverticula and augmentation cystoplasty
Recurrent urinary tract infections
Low fluid intake and dehydration in endemic populations

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Gross hematuria, recurrent UTI, or intermittent urinary stream
  • Acute urinary retention or severe lower-abdominal pain
  • Fever with suspected stone and infection (emergency)

Hydration and treat associated UTI with culture-guided antibiotics

Cystolitholapaxy: transurethral mechanical or laser fragmentation and extraction

Percutaneous suprapubic cystolitholapaxy for very large stones

Concomitant treatment of outlet obstruction (TURP, HoLEP, bladder neck incision)

Catheter care protocols, periodic change, and bladder washouts for high-risk patients

Follow-up ultrasound/KUB 3-6 months after treatment to confirm stone-free status
