What is Bladder Neck Stenosis?

Bladder neck contracture/stenosis is obstruction at the junction of the bladder and urethra resulting from fibrotic changes. In men, it develops after prior prostate surgery (TURP, radical prostatectomy), radiotherapy, or inflammation. In women, it is rarer and most often related to trauma or recurrent infection.

Detrusor overactivity, incomplete emptying, and upper urinary tract damage are late complications of bladder neck obstruction. Cystoscopy provides both diagnostic and therapeutic value.

Transurethral incision (TUIBN) is the primary treatment method; more extensive surgery may be required for refractory cases.

Symptoms

Weak and intermittent urinary stream
Difficulty initiating and prolonged urination
Sense of incomplete emptying and post-void residual
Recurrent urinary tract infections
Incontinence with obstruction after radical prostatectomy
Straining urination in women

Risk Factors

History of TURP or open prostatectomy
Radical prostatectomy (robotic or open)
Pelvic radiotherapy
Recurrent cystoscopy or urinary instrumentation
Recurrent bladder neck infection

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Difficulty urinating after prostate surgery
  • Recurrent urinary infections and sense of incomplete bladder emptying
  • Low maximum flow rate on uroflowmetry (<10 mL/sec)
  • Increased post-void residual urine
  • Lower urinary symptoms together with renal dysfunction

Transurethral bladder neck incision (TUIBN): incision at 5-7 positions with cold knife or laser

Transurethral balloon dilation: temporary solution with higher recurrence risk

Endoscopic laser resection: in narrow circumscribed fibrotic tissue

Open or robotic bladder neck reconstruction: in refractory cases

Urinary diversion: in resistant cases together with bladder dysfunction

Regular urethroscopic follow-up and urinary flow measurement
