What is Benign Prostatic Hyperplasia (BPH)?

Benign prostatic hyperplasia (BPH) is a common men's disease caused by age-related enlargement of the prostate gland pressing on the urethra, resulting in lower urinary tract symptoms (LUTS).

Histological BPH findings are detected in approximately 50% of men at age 50, and in more than 90% of men at age 80. However, not every BPH leads to symptoms.

Severe untreated BPH can cause urinary retention, bladder stone formation, recurrent infections, and kidney damage. Early diagnosis and treatment prevent these complications.

Symptoms

Frequent and urgent sensation of needing to urinate
Frequent nighttime urination (nocturia)
Weak, intermittent, or split urine stream
Difficulty starting urination and hesitancy
Post-void dribbling
Feeling of incomplete bladder emptying
Acute urinary retention (sudden inability to urinate — emergency)

Risk Factors

Advanced age (risk markedly increases after age 50)
Family history of BPH
Obesity and metabolic syndrome
Physical inactivity
Diabetes and cardiovascular disease
High-fat and red meat-dominated diet

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Sudden inability to urinate (acute urinary retention — emergency)
  • Blood in urine (hematuria)
  • Recurrent urinary tract infections
  • Moderate-severe symptoms measured by IPSS score
  • Annual prostate evaluation in men over 50

Lifestyle changes: caffeine and alcohol restriction, evening fluid restriction, bladder training

Alpha blockers (tamsulosin, alfuzosin): relax prostatic smooth muscle to increase urinary flow

5-alpha reductase inhibitors (finasteride, dutasteride): long-term use to reduce prostate volume

Combination therapy: alpha blocker + 5-ARI in patients with large prostate and severe symptoms

Transurethral resection of the prostate (TUR-P): endoscopic partial removal of prostate tissue (gold-standard surgery)
