What is Bariatric Revisional Surgery?

Bariatric revisional surgery encompasses reoperative procedures performed after a primary bariatric operation. Indications include inadequate weight loss, significant weight regain, recurrence of obesity-related comorbidities and mechanical or metabolic complications such as severe gastroesophageal reflux, anastomotic ulcers, hypoglycemia or strictures.

Common revisions include conversion of sleeve gastrectomy to Roux-en-Y gastric bypass, conversion of gastric bypass to one-anastomosis gastric bypass or duodenal switch, band removal with conversion to sleeve or bypass and stoma resizing. Procedure choice depends on initial anatomy, comorbidities, nutritional status and patient preference.

Revisional surgery carries a higher risk of complications than primary procedures, including leak, bleeding, internal hernia and nutritional deficiency. A multidisciplinary preoperative evaluation including dietetic, endocrine and psychologic assessment is essential.
