What is Angle-Closure Glaucoma?

An acute angle-closure attack causes intra-ocular pressure to rise to 40-70 mmHg as the anterior chamber angle suddenly closes. Without urgent treatment irreversible vision loss can occur within hours.

Risk is higher in hyperopic eyes, small anterior segments, older women and Asian ethnicity. Dim lighting, stress and mydriatic drugs can trigger an attack.

Chronic angle closure progresses insidiously as the angle gradually narrows.

Symptoms

Sudden severe eye pain
Headache, nausea and vomiting
Coloured halos around lights
Sudden visual decrease or blurring
Red eye
Mid-dilated pupil unresponsive to light

Risk Factors

Hyperopia and small anterior segment
Older age and Asian ethnicity
Family history of angle-closure glaucoma
Cataract (lens thickening)
Mydriatic or anticholinergic medication
Prolonged time in dim light

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Sudden eye pain, headache and blurred vision — IMMEDIATELY to emergency
  • Sudden halos around lights
  • Red eye combined with vision loss
  • Eye symptoms accompanied by nausea and vomiting
  • Evaluation for prophylactic laser iridotomy when at risk

Emergency intra-ocular pressure reduction with topical and systemic glaucoma medications

Nd:YAG laser iridotomy after the attack subsides
