Androgen-Secreting Tumors

What is Androgen-Secreting Tumors?

Androgen-secreting tumors are rare tumors arising from the ovaries or adrenal glands that secrete excessive amounts of testosterone, DHEA, or DHEAS. They present with rapid-onset hirsutism, virilization, voice deepening, and clitoromegaly in women.

Ovarian tumors include Sertoli–Leydig cell tumors, hilar cell tumors, and ovarian thecomas; adrenal sources include adrenal adenomas and adrenal carcinoma. They must be differentiated from polycystic ovary syndrome and non-classic congenital adrenal hyperplasia.

Serum testosterone >200 ng/dL or very high DHEAS levels suggest an organic cause (a tumor) and warrant advanced imaging.

Symptoms

Rapid-onset hirsutism
Voice deepening
Clitoromegaly
Increased muscle mass and male-pattern body change
Temporal hair recession
Amenorrhea and breast atrophy
Acne and seborrheic skin

Risk Factors

Female sex (peri- and postmenopausal)
Marked androgenic features
History of adrenal or ovarian tumor
Familial endocrine tumor syndromes
Rapid progression unlike PCOS
Virilization developing in pregnancy
Family history of ovarian or adrenal carcinoma

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Rapid-onset hirsutism and virilization
  • Voice deepening and clitoromegaly
  • Very high serum testosterone or DHEAS
  • Peri- or postmenopausal androgenic features
  • Sudden virilization in pregnancy
