What is Anal Cancer: Definitive Chemoradiation with Mitomycin-C and 5-Fluorouracil?

Definitive chemoradiation provides organ-preserving primary treatment for squamous cell anal carcinoma.

Nigro regimen combines external beam radiotherapy with concurrent mitomycin-C and 5-fluorouracil.

Cure rates with chemoradiation exceed 70% in localized disease with permanent colostomy avoidance in 80%.

HPV infection particularly types 16 and 18 strongly associates with anal cancer development.

Salvage surgery (abdominoperineal resection) reserved for residual or recurrent disease.

Symptoms

Anal bleeding, pain, mass, pruritus represent common presenting symptoms.
Tenesmus, change in bowel habits, fecal incontinence may occur with advanced disease.
Inguinal lymphadenopathy may occur with regional spread.
Acute radiation toxicity including dermatitis, mucositis, diarrhea, cytopenias common during treatment.
Late radiation toxicity including fibrosis, fistula, anal stenosis may occur with chronic effects.

Risk Factors

HPV infection particularly types 16 and 18 represents major risk factor for anal cancer.
HIV infection markedly increases anal cancer risk requiring screening in HIV-positive populations.
Receptive anal intercourse and high-risk sexual behaviors increase HPV transmission risk.
Immunosuppression including transplant recipients increases anal cancer risk.
Smoking, prior cervical or vulvar dysplasia or cancer represent additional risk factors.

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Anal symptoms including bleeding, pain, mass warrant urgent evaluation with examination and biopsy.
  • Specialized colorectal or oncology center referral with experienced multidisciplinary teams optimizes outcomes.
  • Severe acute radiation toxicity requires aggressive supportive care and treatment modification.
  • Persistent or recurrent disease after chemoradiation requires evaluation for salvage surgery.
  • Anal cancer screening in HIV-positive populations and other high-risk groups warrants consideration.

Treatment Methods

01
External beam radiotherapy 45-54 Gy to primary tumor and regional lymphatics.
02
Concurrent mitomycin-C bolus on day 1 and 29 with 5-fluorouracil continuous infusion days 1-4 and 29-32.
03
IMRT (intensity-modulated radiotherapy) techniques reduce toxicity while maintaining efficacy.
04
Salvage abdominoperineal resection for residual or recurrent disease after chemoradiation.
05
Comprehensive multidisciplinary care with radiation oncology, medical oncology, colorectal surgery, supportive care during chemoradiation including skin care, nutritional support, antidiarrheal therapy, growth factor support, post-treatment surveillance with examination, anoscopy, imaging, evaluation of long-term sexual and bowel function, salvage therapy planning, HPV vaccination education, HIV management for HIV-positive patients, smoking cessation, and survivorship care addressing chronic radiation effects provides optimal outcomes with cure rates exceeding 70% and organ preservation in most patients with anal squamous cell carcinoma.

Anaemia

Dahiliye (İç Hastalıkları)

Anaemia is a low haemoglobin level that reduces oxygen delivery, causing fatigue, pallor, and shortness of breath. It is not a disease itself but a sign of many underlying conditions. Most cases are correctable with appropriate diagnosis and treatment.

Iron Deficiency Anaemia

Dahiliye (İç Hastalıkları)

Iron deficiency anaemia develops when dietary intake, absorption, or losses create an iron shortfall, most often affecting women and children. Identifying the underlying cause is the core of management, alongside iron replacement.

Vitamin B12 Deficiency

Dahiliye (İç Hastalıkları)

Vitamin B12 deficiency can cause megaloblastic anaemia, neurological symptoms, and cognitive impairment. Early treatment with intramuscular or oral B12 largely prevents irreversible complications.

Hypertension (High Blood Pressure) Management

Dahiliye (İç Hastalıkları)

Hypertension is often called the silent killer because it progresses symptom-free for years and can damage the heart, brain, kidneys, and eyes. Regular monitoring, lifestyle change, and evidence-based drug therapy dramatically reduce cardiovascular risk.

Chronic Kidney Disease

Dahiliye (İç Hastalıkları)

Chronic kidney disease is one of the most common complications of chronic conditions such as diabetes and hypertension, and can be silent in its early stages.

Hepatitis B (HBV)

Dahiliye (İç Hastalıkları)

Hepatitis B is a DNA virus infection causing acute and chronic hepatitis with risk of cirrhosis and hepatocellular carcinoma; diagnosis integrates HBsAg, HBeAg, anti-HBc, and HBV DNA with management based on disease phase using nucleos(t)ide analogues (entecavir, tenofovir) and universal infant vaccination.

Hepatitis C (HCV)

Dahiliye (İç Hastalıkları)

Hepatitis C is an RNA virus causing chronic hepatitis that may progress to cirrhosis and hepatocellular carcinoma; modern direct-acting antiviral (DAA) pangenotypic regimens (sofosbuvir/velpatasvir, glecaprevir/pibrentasvir) achieve sustained virologic response over 95% in 8–12 weeks with universal adult screening and cure for nearly all patients.

Fatty Liver Disease

Dahiliye (İç Hastalıkları)

Non-alcoholic fatty liver disease (NAFLD) is closely related to obesity and metabolic syndrome and is largely reversible with early treatment.
