What is Advanced Oculoplastic Surgery?

Oculoplastic surgery is a subspecialty of ophthalmology focused on plastic and reconstructive surgery of the eyelids, lacrimal system, orbit, and adjacent facial structures. Advanced procedures address complex pathologies: full-thickness eyelid reconstruction following Mohs surgery (Hughes tarsoconjunctival flap, Cutler-Beard procedure, Mustarde cheek rotation flap), orbital decompression for thyroid eye disease (transantral, transethmoidal, lateral wall removal), and external/endonasal DCR for nasolacrimal duct obstruction.

Indications include functional ptosis (visual axis obstruction), severe entropion/ectropion with corneal complications, complicated lacrimal drainage obstruction, anophthalmic socket complications (post-enucleation), Graves' orbitopathy with optic neuropathy or severe proptosis, periocular tumors requiring complex reconstruction, and trauma with orbital floor or medial wall fractures. Multidisciplinary collaboration with ENT (endoscopic DCR, orbital decompression), neurosurgery, and craniofacial teams is common.

Surgical techniques emphasize preservation of eyelid function (blink, closure, lacrimal drainage) and cosmetic outcomes. Modern approaches include endoscopic transnasal DCR, transcutaneous botulinum toxin for entropion, hyaluronic acid filler for tear trough deformity, autologous fat grafting, dermis-fat graft for socket volume, and 3D-printed orbital implants. Functional success measured by tear film stability, eyelid position, and visual rehabilitation.
