What is Advanced Esthetic Tooth Whitening?

Tooth whitening (bleaching) is a chemical process that uses peroxide-based agents to oxidize organic chromophore molecules within tooth structure, lightening both extrinsic and intrinsic discoloration. The two main active compounds are hydrogen peroxide (HP) and carbamide peroxide (CP), where 10% CP releases approximately 3.5% HP. Whitening agents penetrate enamel and dentin via diffusion, breaking down stain molecules and producing colorless byproducts.

Treatment modalities include in-office (chairside) bleaching with high-concentration gels (25-40% HP) applied for 15-20 minutes, sometimes with light/laser activation (LED, halogen, plasma arc, diode laser) intended to accelerate the reaction—evidence for added benefit of light activation is mixed; at-home bleaching with custom-fitted trays loaded with 10-22% CP or 6-10% HP, worn 2-8 hours daily for 2-4 weeks; over-the-counter strips and paint-on gels (lower concentration); and internal bleaching (walking bleach technique) for non-vital, endodontically treated teeth using sodium perborate or HP.

Indications include extrinsic stains (coffee, tea, red wine, smoking), age-related yellowing, mild to moderate fluorosis, and tetracycline staining (limited response, requires extended treatment). Contraindications include pregnancy and lactation (relative), severe enamel/dentin hypersensitivity, active caries or large defective restorations (treat first), active periodontal disease, exposed root surfaces, and unrealistic expectations. Side effects—tooth sensitivity (60-80% of patients) and gingival irritation—are dose-dependent, transient, and managed with desensitizing agents (potassium nitrate, fluoride, amorphous calcium phosphate).
