What is Advanced Endoscopic Sinus Surgery?

Advanced functional endoscopic sinus surgery (FESS) is a high-resolution endonasal technique that opens the natural drainage pathways of the paranasal sinuses, removes diseased mucosa, and treats benign or malignant lesions involving the sinuses and adjacent skull base.

Image-guided navigation, powered microdebriders, balloon dilation, and angled telescopes permit safe approaches to the frontal recess, sphenoid, and pterygopalatine fossa with preservation of normal mucosa.

Indications include severe chronic rhinosinusitis with nasal polyps, recurrent rhinosinusitis after primary surgery, fungal sinusitis, inverted papilloma, juvenile nasopharyngeal angiofibroma, cerebrospinal fluid leak repair, and select skull base tumors.

Symptoms

Persistent nasal obstruction despite medical therapy
Loss of smell (hyposmia or anosmia)
Chronic facial pressure or pain
Recurrent purulent nasal discharge
Postnasal drip and chronic cough
Recurrent sinus infections after prior surgery
Visible nasal mass on examination

Risk Factors

Severe chronic rhinosinusitis with polyps
Allergic fungal rhinosinusitis
Aspirin-exacerbated respiratory disease
Cystic fibrosis
Prior incomplete sinus surgery
Skull base tumor or CSF leak
Inverted papilloma or sinonasal tumor

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Failure of maximal medical therapy for chronic rhinosinusitis
  • Recurrent disease after primary FESS
  • Suspected sinonasal tumor on imaging
  • Persistent CSF rhinorrhea
  • Orbital or intracranial complication of sinusitis
  • Severely impaired quality of life
  • Refractory unilateral nasal symptoms
